HEALTH CENTER

The Rutgers/LEAP Health Center is a partnership between Rutgers University, LEAP Academy, and Cooper Family Medicine that provides access to quality, on-site medical care and support services for children and families at LEAP.  The Center serves as a school-based service hub and seeks to improve academic outcomes by promoting health and positive family engagement in children’s educations. 

The Health Center is housed at LEAP Elementary School and supervised by Chief Physician Dr. Dyanne Westerberg, a medical chief in Cooper Hospital’s Department of Family Medicine.  The Center provides a model for the integration of health and human services in charter schools nationwide, particularly those in medically underserved urban areas. 

SERVICES PROVIDED:

  • Immunizations
  • Referrals to specialists
  • Nutritional counseling
  • Laboratory studies/Blood work
  • Gynecological and prenatal care
  • Well child visits
  • Treatment for Allergies, Asthma, Diabetes, Hypertension


HEALTH CENTER SPECIAL INITIATIVES:

Rutgers/LEAP Family Support Center
The Family Support Center (FSC) is a specialized component of the Rutgers/LEAP Health Center that supplements primary medical care with health education programs, counseling, and behavioral and mental health services.

HOP Clinic for the Uninsured
The HOP Clinic is a special partnership between Rutgers, LEAP, Robert Wood Johnson Medical School, and Cooper Family Medicine to provide free primary healthcare for adults, children, and families in Camden City who lack health insurance.  Third-year medical students at RWJMS staff the clinic, which holds weekly office hours for Adults, Pediatrics, and Women’s health.

INFORMATION FOR PATIENTS

EMERGENCIES
Medical staff is on-call when the Health Center is closed in order to provide phone consultation for urgent medical problems during evening and weekend hours. 

PATIENT PRIVACY
Services at the Health Center are rendered confidentially without regard to race, religion, color, gender, age, sexual orientation, physical or mental disability, or marital, military or veteran's status.

INSURANCE
All health insurance plans are accepted.
*Patients without health insurance are encouraged to visit the Center’s HOP Clinic for the uninsured, which holds weekly evening hours.
